01. Service
We believe individuals should never feel like they are living in someone else’s home, we believe they should feel like it is their home. Our homes promote independence, choice, respect, and belonging while providing individualized supports including assistance with daily living, medication administration, healthcare coordination, transportation, meal planning, independent living skills, community participation, behavioral supports, and goal planning.
02. Service
Community Participation is about creating memories, building friendships, discovering passions, and living life. We plan with individuals; not for them. Every day is built around each person’s interests, strengths, and goals through community exploration, volunteer opportunities, wellness activities, education, recreation, independent living skills, and self-advocacy.
03. Service
We help individuals discover their strengths, prepare for employment, obtain meaningful jobs, and maintain long-term success through career exploration, job coaching, employer collaboration, and ongoing employment supports.
04. Service
Community Integration Services support participation in therapeutic, community-based activities that promote independence, health, wellness, and inclusion. Supports are individualized and based on assessed needs. In accordance with New Hampshire Medicaid Waiver requirements, CIS provides staff support for approved activities but does not pay admission fees, memberships, tuition, or recreational activity costs.
05. Service
We focus on understanding the individual, building upon strengths, and using positive, person-centered behavioral supports that improve quality of life, independence, and relationships.
06. Service
We provide safe, compassionate emergency residential placements while working closely with Area Agencies, hospitals, guardians, and healthcare providers to ensure continuity of care.
07. Service
Our respite services provide trusted, temporary support in a safe and welcoming environment, giving caregivers peace of mind while their loved ones continue receiving high-quality care.
